Full mouth implants rehabilitation

Patient was 45 year old, female, had lost all her teeth at age of 35 due to uncontrolled diabetes

Problem: Patient was 45 year old, female, had lost all her teeth at age of 35 due to uncontrolled diabetes. She was wearing dentures since last 10 years. Now she was fed up of wearing dentures as they constantly got loose, didn’t look or feel natural which had made her lose her social confidence .

Clinical findings– Completely edentulous upper and lower arches. Patient had iron deficiency, B12, D3 deficiency and was menopausal.

Treatment challenge – Bone was poor in quality due to long term denture wear and due to nutritional deficiencies and menopause.

Treatment plan– A procedure with minimal bone drilling was planned and bone condensensation while implant placement was planned to increase the density of the bone surrounding implants. A graftless approach was planned to avoid sinus involvement and grafting.

Procedure – Total 12 implants were placed- 6 implants in upper and 6 implants in lower arch.

Prosthetic rehabilitation – Hybrid prosthesis with PMMA wrap up over metal framework was given to protect and enhance the bone quality.

Final result – We designed the teeth to look like Madhuri Dixit as per patient’s requirement. The patient got fixed teeth that looked and felt just like natural teeth. The patients overall appearance improved to such a extent that patient got feedback from her friends and relatives that she is looking 10 years younger. Her confidence was restored. Even her nutritional deficiencies were resolved once she started chewing food properly with her implant teeth.

Patient was 68 year old, female, unable to eat due to missing teeth in upper left region, and pain in teeth in lower left region.

Problem: Patient was 68 year old, female, unable to eat due to missing teeth in upper left region, and pain in teeth in lower left region.

Clinical findings: Missing premolars and molars, and infected teeth below existing crown and bridge was found. Patient also had history of cough and colddue to allergy tendency.

Treatment challenge: Patient was having deficient bone in area of upper molar. Also due to history of allergy , we wanted to avoid sinus intervention.

Treatment plan: Pain was first addressed and elimated by doing root canals and giving new zirconia crowns and bridges. New bridge was given in lower left region ( and not implants) since the teeth were already prepared by existing bridge, and bone was inadequate for new dental implants. 2 dental implants were planned to restore 3 implant teeth bridge, by using tilted implants to avoid sinus involvement.

Procedure: 2 dental implants were placed , with a angulated multi-unit abutment to ensure proper prosthetic orientation.

Prosthetic rehabilitation: A customised implant abutment was fabricated to make both the implant angulations parallel and the prosthesis passive. A PFM dental implant bridge was designed and fabricated with easy to maintain hygiene spaces with minimum weight yet efficient chewing efficiency.

Final result: The entire left side was restored to maximum possible function , with no pain and efficient chewing capacity.

Immediate implant placement

Patient was 30 year old, male having severe sharp throbbing pain.

Clinical findings: Tooth was grossly carious, and non restorable by root canal. There was periapical lesion on both mesiobuccal and mesiodistal roots.

Treatment challenge: Removal or extraction of tooth should be non- traumatic and causing minimum impact to adjacent bone as we need it to support the new implant. Conserving existing natural bone is always more preferable than creating new artificial bone.

Treatment plan: Removal of tooth by piezoelectric vibration machine for least traumatic extraction and maximum bone preservation. This to be followed by immediate dental implant placement in palatal socket since it was not infected.

Procedure: The tooth was initially sectioned for enabling easy extraction. A piezotome was used to painlessly and atraumatically remove each section. The periapical infection was then curetted. A implant site was prepared in the interdental bone of the palatal bone. The implant with agressive threading and hydrophilic surface was placed in desired site

Prosthetic rehabilitation: A PFM screw- cement retained prosthesis ( SCRP) was designed and execute for the implant cap with single point functional contact and no eccentric contact points.

Final result: Highly satisfied patient since he got a natural feeling tooth in the area of this decayed tooth , without compromising his adjacent natural teeth for a bridge.

Patient was 55 year old, male. He had got 2 dental implant placement in lower left region of jaw in some other clinic around 8 days back, and his pain had not reduced even after suture removal, so he had visited us for second opinion.

Clinical findings: Patient complained of generalised pain. No significant x- ray finding seen. Patient was asked to get CBCT. CBCT showed that placement of implant was improper- not in the centre of bone and perforating the crestal bone integrity.

Challenges: Since the implant was placed only 8 days back, it was not osseointegrated. So removing it was comparatively easier. However, placing implants for same prosthetic requirement in different sites was tricky.

Treatment plan: 2 implants were planned in alternate positions where bone was available , parallel placement of implant was possible while having same prosthetic outcome.

Procedure: Old implants were explanted . The old sites were curetted to ensure infection removal if any. New implants were placed in decided new sites. Proper placement was verified by post- implant CBCT.

Prosthetic rehabilitation: Implants were restored after 3 months with 3 unit splinted zirconia implant bridge .

Final Result: Successful management of a failed implant completed.

Patient was 42 year old, male.He had lost his front 4 teeth . He had initially lost only 1 tooth in a road accident 5 years back, but some local dentist has to do dental bridge by grinding adjacent 3 teeth.

He understood that dental bridge was not a sustainable solutions for him and wanted to get a dental implant done. However, many implantologists had rejected his case since he had very deficient thin bone for implants. So he chose Dr.Mrunal Burute for this complex cases restoration. He visited us from other end of the city ( Sinhagad road) just to get the right treatment this time.

Clinical findings: 4 front tooth missing in upper jaw. CBCT shows extremely deficient thin bone.

Challenge: The bone was extremely thin in width so only narrow implants could fit with a layer of bone on all sides.

Treatment plan: Since diameter of implants had a biological limit, number of implants was increased to give 1 implant per tooth.

Procedure: A ridge- split procedure was done, followed by a bone expansion to place implants in centre of bone .

Prosthetic rehabilitation: 4 PFM teeth( splited to increase strength ) with pink gingival ceramic for precise esthetics was delivered.

Final result: Perfect esthetics were achieved with unrecognisable artificial teeth due to perfect shade matching and tooth form matching

Problem: Patients was 77, male, was missing most of upper teeth and wanted a budget- friendly option to get fixed teeth in upper arch. Patient was a cardiac patient on blood thinners.

Clinical findings: Patient was given a improper full arch bridge over few abutments of temporary acrylic material. Lower arch had few natural teeth remaining which were mobile and carious.

Challenges: Strategic placement of implants to get maximum AP spread to restore teeth as behind as possiblewas challenging. Budget was the primary limiting factor determining the number of implants that we could use for the case.

Treatment plan: 4 implants were planned in All-on-4 configuration with maximum possible diameter and length of implants to maximize bone- implant- contact (BIC).

Procedure: Blood thinners were stopped as per physicians advice , and 4 implants were placed with their respective multi-units abutments . A temporary denture was given to patient for period of 3 months till osseointegration process was completed.

Prosthetic rehabilitation: A PMMA hybrid prosthesis with maximum extension was delivered, while ensuring proper implant occlusion, esthetics and vertical dimension.

Final result: We achieved to deliver budget friendly fixed teeth to patient while restoring his chewing ability.

However for restoration of full upper or lower arches, a hybrid prosthesis which is a combination of PMMA or acrylic wrapped over a metal framework is most suitable from long term life of the dental implants.

All-on-4 is a technique in which the entire upper or lower arch is restored on 4 implants. This is suitable for people who have less bone along the arch such that 5 or 6 implants won’t fit with safe distance between them. The dental implants are strategically positioned at such angles that all vital structures like the sinus or nerve are saved and still posterior teeth can be replaced. They are designed for adults who are missing an entire upper or lower arch of teeth, or those with severely damaged teeth needing full extraction.

All-on- 6 is a technique in which entire upper or lower arch is replaced using 6 implants. The implants position in this case is more straight, equidistant and widespread, allowing a more stronger structure which can enable a wider chewing surface. It is possible in cases where there is abundant good quality bone, or in patients where we can achieve bone till backside by grafting.

This depends on the quality of implant and the implant cap chosen. A basic dental implant cost starts at Rs.21,000/- while a basic acrylic/ PMMA cap starts at Rs.5000/-. A standardized international quality cap can range from Rs.30,000 to Rs.35,000 for the implant, and metal- ceramic cost can go upto Rs.7000.
